About Community First Health Plans, Inc.

Community First Health Plans, Inc. (CFHP) is a non-profit health maintenance organization established in 1995 as a subsidiary of University Health System. It provides health coverage including Medicaid, Medicare, and commercial plans to families, children, expectant mothers, seniors, and individuals with special needs in Bexar County (San Antonio) and seven surrounding counties in South Texas. Because CFHP is a health plan, the information it holds about members can include medical and health insurance records, making a breach particularly sensitive for those affected.

What Should You Do?

If you received notice that your information was involved in this breach, there are several steps you can take to protect yourself. Review any Explanation of Benefits statements from your health plan for services you did not receive, and check your medical records for unfamiliar entries. Consider placing a fraud alert or credit freeze with the three major credit bureaus — Equifax, Experian, and TransUnion — to make it harder for anyone to open new accounts in your name. Request your free credit reports at AnnualCreditReport.com and monitor them for any unfamiliar accounts or activity. If you become a victim of identity theft, report it and get a recovery plan at IdentityTheft.gov.
