According to that filing, the breach occurred on June 25th, 2026, was discovered on July 24th, 2026, and was reported to the Texas Attorney General on August 7th, 2026. One of those earlier notifications described a privacy incident in which protected health information was misdirected to the wrong employer of record due to errors during a transition from a previous financial management services provider. That 2025 incident exposed names, addresses, phone numbers, dates of birth, and Social Security numbers. Those prior incidents are separate from the June 2026 breach reported here.
