About The Detroit News (JOA)

The notice was published on The Detroit News (JOA) website on behalf of Doctor’s Choice Home Care & Hospice, a for-profit home health and hospice provider founded in 2020 and headquartered in Houston, Texas. The organization operates eight locations across northern and eastern Texas, serving the Gulf Coast region with services that include nursing, therapy, hospital-to-home transitions, chronic illness management, and an Advanced Illness Management program. Because Doctor’s Choice provides clinical care to patients, it maintains sensitive personal and health information, including Social Security numbers, clinical records, and scheduling data — all of which were involved in this incident.

What Happened?

The notice published on the organization’s website states that between June 5th, 2026, and July 24th, 2026, an unauthorized party accessed a single clinical user account within the WellSky electronic medical record (EMR) system using compromised credentials. The incident was discovered on July 21st, 2026. Doctor’s Choice’s Chief Technology Officer confirmed that the compromised account was the only point of unauthorized access, and a review of the company’s own internal systems found no related unauthorized activity — the breach was isolated to the WellSky platform. The organization immediately disabled the affected account, notified the City of Houston Police Department (which has an active case assigned), and issued a HIPAA-required public notice on September 19th, 2026, approximately 60 days after discovery. Affected patients are being offered free credit monitoring.

The data exposed includes demographic information (name, address, and date of birth), Social Security numbers, scheduling information, and clinical and treatment information. No ransomware group involvement or threat actor attribution has been reported. As of September 22nd, 2026, no class action lawsuit had been filed, though ClassAction.org has listed a tracking page for a potential Doctor’s Choice Home Care & Hospice Data Breach 2026 class action.

What Should You Do?

If you received a notification from Doctor’s Choice Home Care & Hospice, enroll in the free credit monitoring the company is offering by contacting them at (833) 365-2464 or by mail at 13100 Northwest Fwy, Suite 410, Houston, TX 77040. Because Social Security numbers and clinical information were involved, you should also consider placing a fraud alert or credit freeze with the three major credit bureaus (Equifax, Experian, and TransUnion) and monitoring your credit reports at AnnualCreditReport.com. Review your Explanation of Benefits statements and medical bills carefully and report anything unfamiliar to your health plan or provider.
