About Family Health Centers of Southern Indiana

Family Health Centers of Southern Indiana (FHCSI) is a nonprofit Federally Qualified Health Center that provides primary care, behavioral health, dental care, prescription assistance, and specialty care services to low-income, underinsured, and uninsured residents of Southern Indiana. The organization operates locations in Jeffersonville, New Albany, Corydon, and Clarksville, Indiana, and also runs a mobile dental unit. Because FHCSI serves patients regardless of their ability to pay, it collects a broad range of sensitive personal, medical, and health insurance information from the communities it serves.

What Happened?

Family Health Centers of Southern Indiana was listed in an HHS Office for Civil Rights breach portal report submitted on June 18th, 2026. The incident is classified as a hacking/IT incident affecting a network server. A total of 7,117 individuals were affected. The data involved included names, Social Security numbers, dates of birth, medical information, health insurance information, demographic information, and contact information. FHCSI began mailing notification letters to affected individuals on June 18th, 2026, and published a notice on its website on June 22nd, 2026. A public notice also appeared in the Indiana Exchange on June 25th, 2026.

The ransomware group Termite claimed responsibility for the attack in a February 3rd, 2026 dark web post — more than four months before FHCSI notified affected individuals — and claimed to have exfiltrated approximately 250 GB of data from FHCSI’s systems, including samples posted publicly. The roughly five-month gap between discovery in January and public notification in June may attract legal scrutiny. No class action lawsuit has been filed as of available sources, but at least three law firms — ClassAction.org (sponsored by Bryson Harris Suciu & DeMay PLLC), Migliaccio & Rathod LLP, and Siri & Glimstad — are actively investigating potential class action claims.

Key Facts at a Glance

What Should You Do?

If you received a notification letter from Family Health Centers of Southern Indiana, review it carefully to understand which of your personal details were exposed. Because health information, Social Security numbers, and insurance information were involved, consider placing a fraud alert or credit freeze with the three major credit bureaus — Equifax, Experian, and TransUnion — and monitor your credit reports regularly through AnnualCreditReport.com. Review your Explanation of Benefits statements and request copies of your medical records to check for any services or charges you do not recognize. If you notice suspicious activity, report it to the FTC at IdentityTheft.gov, which can help you create a personalized recovery plan. Keep records of any steps you take in response to this breach.
