What Happened?
Doctor’s Choice Home Care & Hospice was listed in a notice published on the organization’s website on September 19th, 2026. According to that notice, between June 5th, 2026, and July 24th, 2026, an unauthorized party gained access to a single clinical user account within the WellSky electronic medical record (EMR) system using compromised credentials on the WellSky platform. The incident was discovered on July 21st, 2026. The company’s Chief Technology Officer confirmed the unauthorized access was limited to that one account, and an internal review found no related activity on Doctor’s Choice’s own systems. Upon discovery, Doctor’s Choice disabled the affected account, coordinated with WellSky to confirm the scope of the intrusion, and notified the City of Houston Police Department, which has an active case assigned. The company is offering free credit monitoring to affected patients as a precaution.
The information potentially involved includes demographic information such as name, address, and date of birth; Social Security numbers; scheduling information; and clinical and treatment information. No ransomware group claims or independent threat-actor attributions were identified in connection with this incident.
