What Happened?
Next Level Medical, LLC was listed in an HHS Office for Civil Rights breach portal report submitted on September 9th, 2026. The filing describes the incident as a hacking/IT incident affecting information stored on a network server, with a reported count of 501 individuals affected. That figure is a commonly used placeholder when covered entities have not yet completed their review of affected individuals, and Next Level Medical’s notice states the review is ongoing.
According to the company’s own breach notice, as reported by ClassAction.org and ClaimDepot, Next Level Medical first detected suspicious activity on July 11th, 2026, and detected additional suspicious activity on July 29th, 2026. The company engaged third-party cybersecurity specialists to investigate. On August 31st, 2026, the investigation determined that an unauthorized actor had copied certain files from the company’s environment. The company posted a notice on its website and stated it plans to send written notification letters once its review of affected files is complete. The types of information potentially exposed vary by individual and include names, Social Security numbers, dates of birth, medical information, health insurance information, and demographic information.
Separately, the ransomware group known as PEAR claimed responsibility for the attack via a dark web posting observed on approximately August 26th–27th, 2026, alleging it had obtained three terabytes of data. This claim was tracked by Ransomware.live and reported by threat-monitoring sources including BreachSense and GalaxyWarden, the latter of which explicitly noted the PEAR listing is an unverified attacker claim and not a confirmed finding. Next Level Medical’s own notice does not name PEAR or characterize the incident as a ransomware attack. No prior breach history for Next Level Medical, LLC was found in the HHS OCR breach portal or other sources reviewed. No filed class action lawsuit has been confirmed, though ClassAction.org and ClaimDepot have each published investigation pages indicating attorneys are evaluating whether one may be possible.
