About Quantum Health, Inc.
Quantum Health, Inc. is a healthcare navigation and care coordination company headquartered in Dublin, Ohio. Founded in 1999, the company serves employers with self-insured health plans, acting as a single point of contact for healthcare navigation, benefits coordination, and claims support. It works with over 570 employer clients and approximately 3 million members. Because Quantum Health coordinates healthcare benefits on behalf of employers and their plan members, it handles detailed personal, financial, and medical information for a large population of individuals.
What Happened?
Quantum Health, Inc. was listed in a California Attorney General sample breach notice filed on August 14th, 2026. According to the company’s own notification letter and its status page, the incident began on May 29th, 2026, when an employee responded to a fraudulent phone call—a technique known as vishing (voice phishing)—which allowed an unauthorized party to access Quantum Health’s IT network. Between May 29th, 2026 and June 1st, 2026, the attacker accessed and acquired files from certain Quantum Health systems. On June 1st, 2026, Quantum Health detected a service outage, promptly secured and isolated affected systems, launched a forensic investigation with third-party cybersecurity firms CrowdStrike and MOXFIVE, and reported the incident to federal law enforcement. On July 8th, 2026, the investigation confirmed that the compromised files contained personal information. Quantum Health began notifying affected health plan sponsors on July 27th, 2026, and mailed individual notices beginning July 30th, 2026.
The information involved includes names along with one or more of the following: health insurance information (such as insurance policy numbers and claims or benefits information), health information (such as medical information, treatment information, diagnoses, prescriptions, provider names, and dates of service), and other personal information (such as dates of birth, email addresses, physical addresses, phone numbers, and demographic information). The company also stated it could not rule out the possibility that Social Security numbers were involved. The total number of individuals affected nationwide has not been publicly disclosed. The Board of Pensions of the Presbyterian Church (U.S.A.) independently confirmed that approximately 16,000 of its Medical Plan members were among those affected. Reports also indicate that New Era Technology sent notification letters stemming from this same breach, suggesting downstream vendor impact.
This is not the first data security event involving Quantum Health. In a notice dated February 12th, 2026 and filed with the Massachusetts Office of Consumer Affairs and Business Regulation, Quantum Health disclosed a separate earlier incident: an inadvertent disclosure of a claims report on behalf of NTT Health Plan members that occurred between December 15th, 2025 and December 29th, 2025. That report may have included names, addresses, Social Security numbers, member identification numbers, and certain claims information. Additionally, multiple law firms—including ClassAction.org, Dapeer Law, Cole & Van Note, Wilshire Law Firm, and Class Action U—are investigating potential class action lawsuits arising from the May 2026 breach, though no filed class action complaint had been confirmed as of mid-August 2026.

What Should You Do?
If you received a notice from Quantum Health, enroll in the complimentary Kroll identity monitoring services before your activation deadline expires—instructions and your activation code should appear in your notification letter. Regardless of whether you enroll, consider placing a fraud alert or credit freeze with each of the three major credit bureaus (Equifax, Experian, and TransUnion) to make it harder for someone to open new accounts in your name. Review your credit reports at AnnualCreditReport.com, and monitor your existing financial accounts for unfamiliar activity. Because health and medical information was involved in this breach, also review your Explanation of Benefits statements and any medical records for services you do not recognize, which could signal medical identity theft. If you discover signs of fraud or identity theft, visit IdentityTheft.gov for step-by-step guidance on what to do next.
