Data potentially involved includes names, mailing addresses, telephone numbers, email addresses, dates of birth, patient chart or record numbers, appointment dates and times, treating provider information, referral information and related communications, dental X-rays and related imaging, diagnosis, treatment plans and related notes, and insurance and billing information. No ransomware group claim or threat actor has been publicly linked to this specific incident. This practice is a separate and unrelated entity from Park Dental Partners (Roseville, MN) and Park Dental Research Corporation, both of which experienced their own distinct data incidents in 2026.
